Allows Isaac to switch on and off Hunger State.

Cannot be used if Isaac does not have red or rotten hearts.

While in Hunger Mode, Isaac's tears can mark enemies with the Hunger curse.

Isaac can heal himself by eating enemies cursed by the Hunger curse.

Being too long in Hunger State without killing enemies makes Isaac lose health over time. At half a heart, he is forcefully switched to his normal form and cannot use Original Sin until healing.
